Head and Neck cancers have been a major clinical problem in India, which can be in the league of the countries with highest incidence of these cancers in the world. Yet the standards of the care for this dreadful disease have never been uniform and failed short of the international norms leading to a relatively poor survival of these patients across the country. Even though the problem has been prevalent in India more than any country, hardly any research is being carried out in the country both in the clinical or basic sciences aspects. The care of the patients is also distributed among various specialties like general surgeons, Otolaryngologists, surgical oncologists and to some extent maxillofacial surgeons.

Otolaryngologists form a major link in the care of these patients as the primary detection of majority of these patients as well as further care is delivered by them. But it was felt that there is no forum for debate and concerted activities among them to improve the standards of care in head and neck oncology. Also it was felt, when like minded leading clinicians with otorhinolaryngological background from various parts of the country met together, that improving the training as well as standards of care among ENT surgeons to start with will go a long way in making improvements in the dismal scenario. The general feeling was that it will be better to organize the activities among a cohesive group initially and then to expand to include other specialists also. With this objective in mind Dr Ashok Shenoy corresponded to majority of the head and neck surgeons interested in this field and the response was very much encouraging. Following further discussions and a preliminary meeting at Bangalore during the Workshop organized by Dr Tiwari in the Bangalore Institute of Oncology, a national meeting was planned to launch the society in earnest. Dr Sudhir Bahadur organized this preparatory national meeting at the All India Institute of Medical Sciences New Delhi in October 2001.

A national steering committee under the guidance of Dr Ram Mohan Tiwari was formed with Dr Ashok Shenoy as the secretary and Dr Subramania Iyer as the treasurer. Subsequently the society was formed in the name of Foundation for Head and Neck Oncology and the Inaugural meeting was held at Bangalore in January 2002 under the stewardship of Dr Tiwari and Dr Shenoy which was attended by more than 200 delegates from all over the country. The Foundation was registered as a charitable society under the Karnataka Chartable Societies Registration act. Subsequent national conferences were held in Kolkotta, New Delhi and Hyderabad. During all these years the Foundation has been very active in organizing Continuing education programmes, surgical workshops and Postgraduate teaching programmes. Two orations were instituted during the annual conferences, namely Hiranandani oration and PA Shah Oration to honor these doyens of head and neck surgery in India and the orators have been distinguished head and neck oncologists from various parts of the world. From 2006 onwards another oration in the name of Dr RM Tiwari has been instituted to honor a clinical or basic sciences research scientist for his or her contribution in this field.

National Preparatory Meeting
 

Organized by Prof Sudhir Bahadur at the AIIMS New Delhi in October 2001 . A National steering Committee formed to formalize the launch of the Foundation for Head and Neck Oncology. Dr R.M Tiwari, Dr Ashok Shenoy and Dr Subramania Iyer elected as the Founder President, Secretary and Treasurer respectively.
   
First national meeting of the Founder members
  Held in Bangalore on 5-7 January 2002. The present team of office bearers to continue for the term of two years. A national working committee was elected with the following as members- Dr P.Hakim, Dr J.Mody,Dr F.Wadia, Dr K.Ahmed, Dr Behara, Dr P.Hazarika ,Dr P.K.Kakkar and Dr Panda.
   
   
Completion of Registration formalities
  The foundation completed the formalities and was registered as a charitable society under the Karnataka Societies act on 7-2-01. The byelaws were formulated and approved.
   
Approval for Income tax exemption
 

Donations to the foundation was accorded income tax exemption by order Of the Director of Income tax, Bangalore on 16-2-04 . The same has been Extended for a period of three years w.e.f 31-3-05.

   
New team of office bearers
  The new team of office bearers were elected during the national conference held in New Delhi in October 2002.The team included Dr Sudhir Bahadur as the President, Dr Subramania Iyer as Secretary and Dr Madan Kapre as the Treasurer. The national working committee was reconstituted. which comprised of Dr A Thakkar, Dr S.R Patil, Dr Mohan Jagade,Dr Anirudh Dam,Dr Farida Wadia, Dr Ahad and Dr Jatin Mody.

As per the regulations required and forgiving directions to the team a Governing Board was newly constituted with the following as members Dr Ram Mohan Tiwari, Dr Pilloo Hakim, Dr Ashok Shenoy, Dr Ashok Gupta and Dr P.K Kakkar.

The membership rules were amended during the general body meeting in Lucknow to make the fulltime head and neck surgeons from any background to be members of the Foundation. Also notable luminaries in the field from India were honoured by awarding them Invited member status.

Change of the membership rules
  Accepting the necessity for providing as national forum to represent all scientists and clinicians interested in head and neck cancers the Foundation , in its general body meeting held in Mangalore in October 2007. decided to open up full membership to any clinician or basic scientist interested in head and neck oncology.

The 3 rd national Conference was held in New Delhi in October 2003. The organizing team was led by Prof Sudhir Bahadur . The overseas faculty included Dr A.Balm (P.A.Shah oration) Dr U.Prasad ( Hiranandani oration) and Dr Giovanni. Regional workshops and CME programmes were conducted in King George Medical University Lucknow ( Dr Naresh Bhatia) , Kailash Cancer Center, Goraj Baroda ( Dr Atul Shah) Grant Medical College Mumbai (Dr Mohan Jagade), Chikildara, Nagpur(Dr Madan Kapre) Kidwai Institute of Oncology Bangalore (DrAshok Shenoy).

 
YEAR 2005
 
The 4 th national conference was held in Hyderabad on 4-6 January under the stewardship of Dr PSN Murthy, Dr Sridhar Reddy and Dr T.B Ramakrishna. The overseas faculty included Dr Piero Nicolai (Hiranandani Oration) Dr Guiseppe de Maria ( P.A Shah Oration) , Dr Franz Hilgers, and Dr Patrick Bradley. The FHNO gold medal for best paper and poster was initiated in the meeting. CME programmes/ surgical workshops were conducted in the Command Hospital New Delhi (Dr Sukthanker ),Cancer Institute Nagpur ( Dr Sanjeev Golhar) Chikildara, Nagpur (Dr Devan Mahore) Gowhati (Dr Kuddush Ahmed). A three day PG teaching programme was also held in the AIMS, Kochi.
 
Year 2006
 

The 5th national Conference was held at the King George Medical University  Lucknow on 3-5 January 2006 with Prof Naresh Bhatia as the organizing secretary. The faculty included Prof Patrick Gullane (Hiranandani oration), Dr Peter Rhys Evans (PA Shah oration) and Dr Wesley Hicks and DR Cherrie Ann Nathan. The CME programmes during this year were held in Kochi, Nagpur, Srinagar Goraj and Mangalore.

The 6th national conference was held on 27-29th October 2006 in Udaipur with Prof Ashok Gupta and Dr Bhuie comprising the  organizing team.The overseas orators included Dr David Howard ( PA Shah oration) DR Kowalski ( Hiranandani oration), Dr Carter Van Waes ( Tiwari research oration)  Dr Piero Nicolai and Dr Guiseppe Spriano.

 
Year 2007 
 

The 7th national conference was held in Mangalore under the stewardship of Dr Kishore Chandra Prasad on 26-28 October 2007. Overseas faculty included Prof Steiner ( Hiranandani oration ) Dr Rick Heyden ( PA Shah oration) DrVan Del Breckel(Tiwari oration)               

The CME programmes were conducted during this year in Kochi, Nagpur, Chikildara, Manipal ,Goraj and  Rourkela

 
Milestones in Development- Training programmes and Research Activities
 
Initiation of first structured training programme in Head and Neck oncology
 

December 2003 -A three year structured training programme was started by the Amrita Institute in Kochi culminating in award of MCh degree by the Amrita Vidyapeeth (deemed university) .The same curriculum was submitted to the National Board for starting a fellowship-training programme

Similar programmes are planned to be initiated by the Rajeev Gandhi Medical University, Bangalore in 2006. Initially a 1-½ year programme and then a 3-year full-fledged programme is planned to be started.
   
Institution of short term training fellowships
 
Two training fellowships of 6 months duration has been set up by the Bangalore ENT trust in head and neck Oncology from July 2004. This planned to be extended to one-year fellowship from this year onwards.
   
Formation of Multi center research group in head and neck oncology
 

A national cooperative research group was started in December at the culmination of a preparatory meeting of Head and Neck oncologists from various parts of the country held in Amrita Institute, Kochi with Dr Jay Boyle of Memorial Sloan Kettering as the resource person. The group was named HN-COG (Head and Neck Co operative research group)

   
Institution of Research Oration
 
From 2006 a separate oration in Research has been instituted, to be delivered in the national conference. The orator will be an outstanding research scientist from India or abroad and in recognition of the work the orator will be presented with the FHNO gold medal in research. The oration and gold medal has been set up with the benevolence of Dr R.M Tiwari.
 
Initiation of   Shireen Gazdar Fellowship
 

With the benevolence Ms Spencer and the efforts of Dr Pilloo Hakim a long term ( 1 year) fellowship training programme was initiated from year 2007.
